PLEASE BE SURE TO NOTIFY A RECEPTIONIST
WHEN YOU ARRIVE AND INFORM THEM OF ANY RECENT CHANGES TO YOUR ADDRESS, PHONE NUMBER, HIPPA CONTACTS OR INSURANCE
Office visits are by appointment only. Please call the office in advance to schedule a date and time. If you develop an urgent problem, please call the office as soon as possible so that we can make every effort to accomodate your needs. We ask that you arrive 15 minutes prior to your scheduled appointment to allow time for updating
your contact information, address, insurance, etc.
If you can not keep an appointment, please notify us as soon as possible. You may be charged a fee if office visits are not cancelled at least 24 hours prior to a missed appointment.
Walk-in visits are discouraged. Walk-in visits create schedule issues for the office staff. We prefer, if you have an emergency, that you CALL FIRST so we can advise you on what to do to ensure that you will receive the most appropriate care. Emergencies may best be handled in another setting (eg. Emergency Room) rather than the office.
If you are more than 15 minutes late for your appointment you may be asked to reschedule so as not to disrupt previous scheduled appointment times.
Our phones are open during routine office hours and are turned off between the hours of 12pm and 1pm for the lunch hour.
A nurse is available for phone consultation every day. In general,
doctors cannot interrupt patient visits to answer your phone calls.
If you absolutely need to speak with a provider, please call and leave a message with one of our receptionists and your provider will return your call when he/she is able. Otherwise, please feel free to discuss your matter with a Registered Nurse, who will consult with your provider and
offer you professional and timely advice.
Keep in mind, good medical care can not always be accomplished
over the telephone. When necessary, a nurse will advise you to
schedule an office visit to discuss your concerns, problems or test
If you had blood work done or tests ordered by one of our providers,
a Registered Nurse from our office will contact you when those results are available and have been reviewed by your provider. Our office is not responsible for notifying you of test results that have been ordered by outside physicians.
If you would like a copy of your results sent to you, please let
us know and we would be happy to mail you a copy.
Due to New York State HIPPA laws, we are not able to leave your information on an answering machine or voice mail to protect your privacy. If you have received a message from our office, please return
the call in a timely manner, as your results and/or medication changes may be time sensitive.
Fees, co-payments, and insurance deductibles are due and payable at the time of service. If your children are being seen, be sure they are
prepared to pay as well. Payment by cash, check, or credit card will be accepted. At the end of your visit, you will be given a form showing the fees for your visit and any prior balance. If you have questions about your bill, please call and discuss them with our billing department.
Health insurance is a contract between the patient and the insurance
company. We will provide you with all the information necessary for you to get reimbursement from your insurance company.
In the case of prolonged illness, large bills, or other financial difficulties, monthly payment schedules can be arranged to allow you to pay off balances due in equal payments over a specific time period.
WE ASK THAT YOU PLEASE SILENCE YOUR CELL PHONE WHILE YOU ARE AT OUR OFFICE AND DURING YOUR APPOINTMENT.
